You can submit either a paper copy of the below PDF by filling out and mailing it to the HOA's ARC review committee OR you can submit the digital ARC Request form below. There is no need to do both.
A minimum of 2 weeks must be allowed to approve the form.
The physical mailbox is checked about once a month, so waits may be longer if a paper form is sent instead.
